This material may also appear on the FGSL Facebook page or other social media pages. Web and social media site addresses that FGSL maintains will be given to parents upon request. This authorization is continuous and may only be withdrawn by my specific rescission of this authorization. Consequently, FGSL may publish materials, use my name, photograph, and/or make reference to me in any manner that FYSL deems appropriate in order to promote/publicize the organization.

I do hereby waive, release, absolve, indemnify and agree to hold harmless Fowler Girls Softball League, Fowler Unified School District, the organizers, coaches, sponsors, officials, supervisors, other participants and appointed persons transporting my child to and from program activities for any claims arising out of injury to my child incidental to such participation except to the extent and amount covered by accident and/or liability insurance help by Fowler Youth Softball League.
